Medical Recliner Evaluation Agreement
KindCare Medical appreciates your interest in our Medical Recliners. We understand that you or your customer must give careful consideration to the purchasing and selection process.
A “Hands On” evaluation of our products is very beneficial when making these important purchasing decisions.
•KindCare Medical will pay for the freight from our plant to the ship to address (additional services i.e. lift gate, inside delivery are the responsibility
of the recipient).
•You are responsible for return freight ONLY, if you decide to return our product(s).
•The product(s) are available for evaluation for a period of 10 business days AFTER receipt of the product.
•The Medical Recliner IS NOT to be subjected to regular use as a substitute or replacement unless you or your customer commits to a purchase.
•Please contact KindCare Medical after 5 business days to confirm the disposition of the product. There will be a 30% restocking fee for any
product not returned on a timely basis. KindCare Medical will not automatically arrange for pick up.
•KindCare Medical provides the proper carton and packing materials when a product is shipped. We ask that you save and USE THE SAME carton and materials
when returning the product. Please repack the product carefully so no damage will occur during return transit.
You will be charged for a new box, plus freight, if you discard the original box and packing material.
•You agree to pay for any damage to the product (other than shipping) such as torn upholstery, broken parts or any work required refurbishing the product to
a like new condition.
